This retrospective study from Kyungpook National University evaluated the efficacy of consolidative hypofractionated radiotherapy (RT) in 216 patients with oligometastatic non-small cell lung cancer (NSCLC) treated between 2013 and 2024. Hypofractionated RT was delivered with biologically effective doses (BED) ≥50 Gy in under 10 fractions targeting 267 lesions, predominantly in the lung.<br /><br />Key outcomes measured included progression-free survival (PFS), local control (LC), widespread progression-free survival (WS-PFS), overall survival (OS), and toxicity. At one year, LC was 87.3%, PFS 51.2%, WS-PFS 62.6%, and OS 80.0%. Median PFS was 1.0 year and median OS was 2.6 years, indicating consolidative RT can offer meaningful disease control and survival benefit in oligometastatic NSCLC.<br /><br />Multivariable analyses showed that traditional tumor characteristics such as site, size (gross tumor volume), number of lesions, and RT dose did not significantly impact survival outcomes. Instead, the oligometastasis status—whether genuine or induced—was a strong predictor of clinical outcomes (with genuine oligometastasis associated with better PFS and LC). Other favorable factors included initial stage, initial treatment modality, and concurrent chemotherapy during RT.<br /><br />Treatment-related toxicity was low, with only 6% experiencing grade 2 or higher radiation pneumonitis and 1.8% grade 3; no grade 4 or worse events occurred, underscoring tolerability of the hypofractionated RT regimen.<br /><br />In conclusion, consolidative hypofractionated RT for oligometastatic NSCLC provides good local control and promising survival, with oligometastasis status being a key factor in prognosis. These findings suggest personalized treatment strategies considering oligometastatic status may optimize outcomes.
