This study explores the application of Multi Criteria Decision Analysis (MCDA) as an evidence-based Health Technology Assessment (HTA) method in healthcare decision-making in Egypt, specifically for mesothelioma treatment. MCDA enables simultaneous assessment of multiple criteria—designed to be complete, non-redundant, and mutually independent—facilitating informed choices among drug alternatives.<br /><br />Researchers developed an MCDA framework incorporating 11 criteria related to medication context, drug outcomes, economic factors, and quality of evidence. Each criterion was scored on a scale from 1 to 3. A Delphi study was conducted to refine this criteria list, ensuring clear definitions and an appropriate scoring scale. An appraisal group then individually assigned weights to each criterion, reflecting their perspectives, before scoring the performance of two drugs: pemetrexed and gemcitabine. These inputs were combined using normalized weights and scores to calculate an overall value estimate for each medication.<br /><br />The results yielded a preliminary MCDA rating tool tailored for mesothelioma management in Egypt. The total aggregate scores, computed by applying average weights and scores to each criterion, showed pemetrexed scoring 2.05 versus gemcitabine’s 1.99. This indicates a slight preference for pemetrexed based on the combined assessment of multiple relevant factors.<br /><br />In conclusion, the study demonstrates that applying MCDA in mesothelioma treatment decisions supports the selection of pemetrexed over gemcitabine, providing a structured, transparent, and evidence-based approach to healthcare decision-making in the Egyptian context.
