EP.14.10 Role of CT Assessed in the Evaluation of Sarcopenia in Patient With Malignant Pleural Mesothelioma. Results From an Observational Study

This observational prospective study evaluated the prognostic significance of sarcopenia, assessed via computed tomography (CT), in patients with malignant pleural mesothelioma (PM) undergoing first-line treatment with chemotherapy or immunotherapy. Sarcopenia, defined by a skeletal muscle index (SMI) at the L3 vertebral level below 39 cm²/m² for females and below 52 cm²/m² for males, was quantitatively measured from baseline CT scans in 48 patients enrolled in the Micromeso study.<br /><br />Patient demographics included a median age of 72 years, predominance of males (38/48), and most had epithelioid histology (34/48). First-line treatments were predominantly carboplatin-pemetrexed (32 patients) or ipilimumab-nivolumab (16 patients). Median follow-up was 15 months, with median overall survival (OS) at 15 months for the entire cohort.<br /><br />Comparing sarcopenic versus non-sarcopenic patients, median OS was 13 months (95% CI: 10.5–17) versus 17 months (95% CI: 9.8–17.1), respectively, with no statistically significant difference (p=0.13). Progression-free survival (PFS) also showed no significant difference: 6.4 months in non-sarcopenic versus 9.5 months in sarcopenic patients (p=0.28). Although there was a nonsignificant trend toward longer survival in non-sarcopenic patients, sarcopenia did not emerge as a definitive prognostic factor in this cohort.<br /><br />The study concludes that CT-assessed sarcopenia alone may not be a reliable prognostic biomarker in PM patients treated with first-line therapies. Ongoing analyses aim to further investigate how body composition changes during treatment correlate with response and survival outcomes. These findings highlight the need for additional research into sarcopenia’s role within the broader context of tumor biology and patient management in pleural mesothelioma.
