EP.17.11 Integrating Prevention and Screening of Tobacco-Related Diseases Including Lung Cancer Screening: Results of a Pilot Program

This pilot study evaluated an integrated clinical program at Hôpital Nord in Marseille, France, combining smoking cessation, lung cancer screening (LCS), and prevention and screening of other tobacco-related diseases (TOREDI) such as cardiovascular disease (CVD) and chronic obstructive pulmonary disease (COPD). Conducted from November 2023 to December 2024, the retrospective monocentric study included structured smoking cessation (both in-person and remote), cardiovascular risk assessment, spirometry for COPD symptoms, and LCS based on eligibility.<br /><br />Out of 346 planned first visits, 300 occurred, with 244 (86.2%) patients identified as active smokers. A total of 965 follow-up visits were completed, predominantly nurse-driven (57%), with 42% by physicians; 58% of visits were conducted remotely via phone. Patients attended a median of five visits, achieving smoking cessation after a median of 120 days.<br /><br />Cardiovascular and metabolic screening of 290 patients revealed notable prevalences: atheromatous cardiovascular disease (28.3%), hypertension (26.9%, with 7.2% uncontrolled), dyslipidemia (30.7%, 5.5% uncontrolled), and diabetes (10.3%, 2.8% uncontrolled). Regarding pulmonary screening, 29.3% had a history of cancer (7.6% active), 47.9% underwent LCS, spirometry was performed in 44% with unknown COPD, discovering airway obstruction in 10.6% (Tiffeneau index <70%).<br /><br />The study concludes this integrated, mainly nurse-led program incorporating remote consultations is feasible and effective in identifying uncontrolled or previously undiagnosed tobacco-related risk factors and conditions. The findings support expanding this model to multicenter settings and developing standardized care pathways to improve comprehensive tobacco-related disease management beyond lung cancer screening alone.
