EP.17.36 Disparities in Survival Outcomes of Primary Pulmonary Diffuse Large B-Cell Lymphoma: A SEER Database Analysis

This population-based retrospective cohort study analyzed survival disparities in primary pulmonary diffuse large B-cell lymphoma (DLBCL) using SEER data from 2000 to 2021, comprising 1,524 adult patients aged 20 and older. Primary pulmonary DLBCL is a rare extranodal lymphoma of the lung, accounting for only 0.5–1% of primary pulmonary malignancies, and survival factors remain underexplored. The cohort was predominantly older (65 years or above in 65%), female (51%), and White (75%), with 38% presenting metastatic disease at diagnosis. Most patients lived in metropolitan areas and nearly half had median household incomes below $75,000.<br /><br />Key findings indicated that older age (≥65 years), presence of metastatic disease, and unmarried status were significantly associated with worse overall survival (OS). Median OS was dramatically shorter in older patients (30 months) versus younger (171 months), and among those with metastatic disease (45 months) compared to localized or regional stages (83 and 94 months, respectively). Unmarried patients also had poorer outcomes (median OS 41 months) relative to married patients (82 months). Conversely, race/ethnicity, income level, and urban versus rural residence showed no statistically significant impact on survival.<br /><br />These results highlight the prognostic importance of age and social support, emphasizing the need for early psychosocial risk screening and tailored supportive care interventions to improve outcomes, especially in older and socially vulnerable patients. The study recommends further research into personalized geriatric and psychosocial interventions, exploration of molecular/genomic factors underlying disparities, and validation through multicenter data to inform targeted treatment strategies. Overall, these insights underscore the multifaceted influences on survival in this rare lymphoma population beyond traditional clinical factors.
