EP.18.01 The Beneficial Value of 18F FDG PET/CT in the Follow-Up of Stage III Non-Small Cell Lung Cancer Patients (NVALT31-PET Study)

The NVALT31-PET study is an ongoing multicenter randomized controlled trial investigating the benefit of [F] FDG PET/CT compared to conventional CT surveillance during follow-up of stage III non-small cell lung cancer (NSCLC) patients treated with curative intent. Despite advances in treatment, stage III NSCLC patients have a high recurrence risk of 50-78%, including local recurrence (22-50%) and distant metastases (48-78%). Early detection of oligometastases is critical for potential curative re-treatment.<br /><br />The primary aim is to assess the impact of PET/CT versus CT surveillance on 3-year overall survival. Secondary objectives include evaluating recurrence detection rates, health-related quality of life, treatment types after recurrence, fear of cancer recurrence, event-free survival, the role of circulating tumor DNA (ctDNA) in recurrence detection, and cost-effectiveness.<br /><br />The trial randomizes 690 patients 1:1, stratified by histology and treatment type, with intention-to-treat analysis planned using Kaplan-Meier survival curves. Preliminary baseline data from 237 patients show comparable groups in age (~67 years), sex distribution, ECOG performance status, histology (majority adenocarcinoma and squamous cell carcinoma), and primary curative treatments (mainly chemoradiation with or without immunotherapy, surgery, and radiotherapy).
