P1.04.10 Lung Cancer Screening Completion Rates as Predicted by Social Deprivation

This study examined how social deprivation predicts lung cancer screening (LCS) completion rates among primary care patients in a large U.S. health system. Lung cancer disproportionately affects individuals with lower socioeconomic status (SES), who also tend to have lower LCS uptake, despite guidelines recommending low-dose CT screening for high-risk groups. Using electronic health record data from September 2022 to August 2023, the researchers analyzed 829 patients referred for LCS across 51 clinics. Social deprivation was measured using the 2019 Social Deprivation Index (SDI), categorized into four groups from least (1-24) to most deprived (75-100).<br /><br />Overall, only 24% of referred patients completed LCS, demonstrating low adherence. The sample was socioeconomically diverse, with roughly a quarter in the lowest and nearly a third in the highest deprivation categories. Adjusted logistic regression with generalized estimating equations, controlling for sex, race, smoking status, insurance, and clinic type, revealed that patients in the highest SDI category had significantly lower odds (OR = 0.58, 95% CI: 0.36-0.94) of completing LCS compared to those in the lowest deprivation category. Other demographics showed differences by completion status, such as race and smoking status.<br /><br />The study concludes that higher social deprivation is associated with reduced completion of lung cancer screening after referral, highlighting the need for targeted interventions beyond referrals to increase screening uptake among socioeconomically disadvantaged groups. The authors note the limitation of using a single social deprivation measure and call for additional strategies to address barriers impacting successful LCS completion in lower SES populations. This research underscores the persistent disparities in lung cancer prevention linked to social determinants of health.
