P1.04.19 Evaluating the Impact of Electronic Reminders on Lung Cancer Screening

This study evaluated the impact of an electronic health record (EHR) reminder system, called Health Maintenance Due (HMDue), on lung cancer screening (LCS) rates at Stanford University and affiliated sites. Lung cancer screening reduces mortality among heavy smokers but is underutilized, with low screening rates nationally and at this institution. The HMDue program, implemented in May 2022 within the Epic EHR system, assigned eligible patients—aged 50-80 years, with a 20 pack-year smoking history, currently smoking or quit within 15 years—one of five screening statuses (Not Due, Due Soon, Due On, Overdue, Discontinued). Screening status was monitored every three months over the course of one year.<br /><br />At rollout, 8,346 patients met LCS criteria, with 91% categorized as Overdue for screening. Within one year, 14% of the Overdue group completed screening, leading to a significant increase in overall screening rates from 9.3% to 16% (p<0.001). Logistic regression identified factors associated with higher screening completion, including Black race (OR 1.43), part-time or unemployed status (OR 1.48), and family history of cancer (OR 1.97). The Sankey plot depicted transitions in screening status over time, showing gradual improvements but persistent majority overdue.<br /><br />The findings demonstrate that implementation of the HMDue electronic reminder system is a low-burden, easily scalable intervention that significantly improved lung cancer screening rates within one year. Despite this progress, overall screening rates remain suboptimal, highlighting the need for continued efforts leveraging digital tools and system-level strategies to further enhance early detection of lung cancer among high-risk populations.
