P1.07.27 A Newly Developed 3D Simulation System for the Preoperative Planning for Central-Located Lung Cancer

This study evaluated Bioxel, a novel 3D simulation system designed to enhance preoperative planning for surgeries of centrally located lung cancer, which traditionally pose challenges due to tumor proximity to critical hilar structures. Conventional CT imaging often lacks sufficient spatial detail for precise surgical planning in such cases.<br /><br />The study had two parts. In the first, an experienced surgeon used Bioxel for preoperative planning in 50 cases and traditional CT-based planning in another 50. In the second part, a different surgeon planned 100 cases with CT alone, then re-evaluated them with Bioxel. Key metrics assessed were tumor invasion assessment accuracy, surgical duration, intraoperative blood loss, and positive surgical margin rates.<br /><br />Results showed that Bioxel significantly improved tumor invasion assessment accuracy from 78% with CT alone to 89% with Bioxel, reaching 94% accuracy when combining both methods. Surgical duration decreased from an average of 216.2 minutes with CT planning to 196.8 minutes using Bioxel. Blood loss was reduced from 301.2 mL to 158.2 mL, and positive margin rates declined from 8% to 2%.<br /><br />In conclusion, Bioxel provides detailed 3D reconstructions that improve surgical precision and safety in centrally located lung cancer resections. It reduces dependence on invasive techniques like bronchoscopy and has the potential to optimize surgical outcomes and streamline clinical workflows. These findings support the integration of advanced 3D simulation systems such as Bioxel into preoperative planning protocols to improve patient care in complex lung cancer surgeries.
