P1.11.58 Efficacy of First-Line Treatment in Driver-Negative Non-Small Cell Lung Cancer With Liver Metastases : A Bayesian Network Meta-Analysis

This Bayesian network meta-analysis evaluated optimal first-line treatments for driver gene-negative non-small cell lung cancer (NSCLC) with liver metastases to guide clinical decisions. The study systematically reviewed Phase III randomized controlled trials, including 15 trials with 1,237 patients, comparing PD-1/PD-L1 inhibitor-based therapies or combination anti-angiogenic therapy against chemotherapy alone. The primary endpoint was progression-free survival (PFS); overall survival (OS) was secondary.<br /><br />Results indicated that combining a PD-1 inhibitor with chemotherapy significantly improved both PFS (hazard ratio [HR] 0.57) and OS (HR 0.68) relative to chemotherapy alone. This benefit was consistent across NSCLC histologies, with HRs of 0.37 for non-squamous and 0.58 for squamous subtype. Among 11 treatment strategies assessed, camrelizumab plus chemotherapy ranked highest, with a surface under the cumulative ranking curve (SUCRA) value of 83.06% for PFS and 96.38% for OS, suggesting superior efficacy.<br /><br />In conclusion, for treatment-naive patients with driver-negative NSCLC harboring liver metastases, the addition of a PD-1 inhibitor to chemotherapy provides enhanced survival outcomes over chemotherapy alone. Notably, camrelizumab plus chemotherapy emerged as a potentially optimal regimen, offering substantial improvements in both progression-free and overall survival.
