P1.17.14 Regional Disparities in Late-Stage Lung Cancer Outcomes in Sweden - Insights From Population-Based Data

This study investigates regional disparities in late-stage (stage IV) lung cancer outcomes across Sweden using population-based data from 2014 to 2022, sourced from the Swedish Cancer Registry. Lung cancer patients totaled 37,206 during this period, with nearly half diagnosed at stage IV. The majority were elderly (mean age 71) and women (53%). <br /><br />Age-standardized incidence rates of stage IV lung cancer varied significantly across counties, ranging from 14 to 28 per 100,000 person-years. One-year overall survival (OS) and relative survival (RS) rates also showed regional differences, with OS ranging from 26.7% to 37.2% and RS similarly variable between counties and six broader healthcare regions. Mortality rates in 2022 ranged between 16 to 24 per 100,000 person-years, generally declining over time but with notable regional variation.<br /><br />The high proportion of late-stage diagnoses underscores an urgent need for earlier detection strategies and improved, equitable access to timely lung cancer care across Sweden. The observed disparities in incidence, survival, and mortality highlight regional differences in healthcare delivery and outcomes. Equal access to targeted interventions and regional quality improvements could help reduce these disparities and improve prognosis for lung cancer patients.<br /><br />The analysis was performed using the VizCan interactive platform, which compiles population-based cancer data across 18 cancer types and is publicly accessible. This platform facilitates an enriched understanding of factors contributing to variations in lung cancer care and outcomes and supports efforts to enhance cancer management in Sweden.<br /><br />In conclusion, while overall lung cancer mortality is declining, significant regional disparities in late-stage lung cancer diagnosis and outcomes exist in Sweden. Addressing these through earlier diagnosis and ensuring equitable healthcare access is critical to improving survival and reducing mortality rates nationwide.
