P2.15 .11 Non-Cancer-Related Mortality in Lung Cancer Patients: The Role of Comorbidities in a 7-Year Retrospective Case-Control Study

This retrospective case-control study (2016-2023) investigates the role of comorbidities in non-cancer-related mortality among 503 lung cancer patients treated at Fundación Santa Fe de Bogotá, Colombia. Lung cancer remains the leading cause of cancer death globally, and chronic comorbidities are known to worsen prognosis by complicating treatment and increasing mortality. However, non-cancer causes of death are often underreported in this population.<br /><br />Data collected included sociodemographic factors, tumor characteristics, treatments, and comorbid conditions—primarily cardiovascular, respiratory, metabolic, and thromboembolic. Mortality within two years was analyzed, distinguishing between cancer- and non-cancer-related deaths using multivariate logistic regression adjusted for tumor stage, age, sex, and treatment type.<br /><br />Key findings revealed that 25% of deaths among lung cancer patients were due to non-cancer causes. Chronic obstructive pulmonary disease (COPD) and pulmonary embolism emerged as independent predictors of increased mortality, underscoring the critical need for early diagnosis, prevention, and active management of these conditions. Interestingly, heart failure was associated with lower mortality, potentially reflecting selection bias or compensated cardiac function in the cohort. The study highlights that integrating cardio-oncology and pulmonology care could mitigate adverse outcomes by better managing comorbidities.<br /><br />The authors recommend embedding comorbidity control into standard lung cancer care protocols. Future prospective research should evaluate interventions focused on comorbidity management, aiming to improve survival and quality of life for lung cancer patients. This study contributes important evidence that addressing non-cancer health issues is vital alongside cancer treatment to reduce overall mortality in lung cancer populations.
