P2.15 .19 Assessment of NSCLC Treatments and Correlation With Hypogonadism in Male Patients With NSCLC

This retrospective cohort study assessed hypogonadism prevalence and its association with NSCLC treatments in 4,428 adult males using the TriNetX database. Hypogonadism was defined as total testosterone under 300 ng/dL. At baseline, 34.1% (1,507) of patients had hypogonadism; they were older (mean 67.3 vs. 64.1 years) and had higher BMI, with more comorbidities like diabetes (34.3% vs. 20.8%) and hyperlipidemia (63.3% vs. 45.9%) than those without hypogonadism.<br /><br />The study evaluated changes in testosterone pre- and post-therapy and employed logistic regression to analyze hypogonadism risk relative to treatments including chemotherapy, immunotherapy, and targeted therapy. Results showed targeted therapy posed the highest odds of both new hypogonadism onset and worsening existing hypogonadism. Chemotherapy had a modest association with new hypogonadism development, whereas immunotherapy demonstrated no significant link.<br /><br />Key takeaways include that over one-third of men with NSCLC have hypogonadism at diagnosis, and targeted treatments contribute most significantly to endocrine dysfunction. Immunotherapy appears least associated with hypogonadism risk. The study highlights that hypogonadism remains an underrecognized complication in NSCLC management amid improvements in survival outcomes driven by systemic therapies.<br /><br />Authors recommend prospective studies to clarify benefits of routine endocrine screening and potential testosterone replacement for these patients, aiming to improve quality of life, reduce fatigue, and maintain functional status. Current findings underscore the importance of monitoring hormonal health in men undergoing NSCLC treatment, especially those receiving targeted agents, to address this prevalent yet often overlooked clinical issue.
