Pdf Summary
This study by Dr. Raymond Osarogiagbon at Baptist Cancer Center evaluated the impact of structured multidisciplinary care (MDC) on overall survival (OS) among lung cancer patients diagnosed through lung cancer screening (LCS) and incidental pulmonary nodule (IPN) programs within a regional community healthcare system (2015–2024). The research cross-linked patient data from LCS and IPN programs with an institutional MDC database, analyzing differences in demographics, clinical characteristics, and survival outcomes, adjusting for factors such as age, sex, race, smoking history, insurance, rurality, and cancer stage.

Among 2,526 patients diagnosed through these early detection programs (613 via LCS, 1,913 via IPN), 41% received structured MDC. Key findings included younger median age and higher rates of early-stage (I/II) diagnosis and surgical resection in the MDC cohorts. Histologic differences were noted with adenocarcinoma more common and small cell lung cancer less frequent among MDC patients.

Importantly, OS was significantly improved in patients receiving MDC across both programs. Five-year OS rates were higher with MDC: 65% vs 54% in LCS, 48% vs 35% in IPN, and 51% vs 39% combined. Adjusted hazard ratios for death were significantly elevated for patients without MDC, and these findings remained consistent after propensity matching to account for potential confounders.

The study concludes that embedding structured, multidisciplinary decision-making within early lung cancer detection programs leads to markedly better survival outcomes. This emphasizes the clinical value of coordinated, guideline-concordant care in improving prognosis in early-detected lung cancer patients.
