This study analyzed the global burden of tracheal, bronchus, and lung (TBL) cancer deaths, disability-adjusted life years (DALYs), and years lived with disability (YLDs) attributable to high fasting plasma glucose (HFPG) across G20 countries from 1990 to 2021 using data from the Global Burden of Disease Study 2021. TBL cancer is a leading cause of cancer mortality worldwide, primarily linked to tobacco, but HFPG—associated with diabetes and prediabetes—is increasingly recognized as a significant risk factor.<br /><br />Between 1990 and 2021, the absolute burden of TBL cancer related to HFPG escalated dramatically: deaths rose by 162%, DALYs by 132%, and YLDs by 194%. Indonesia, India, and Saudi Arabia experienced the largest increases in deaths (456%, 350%, and 336%, respectively), while the United Kingdom had the smallest rise (43%). By age, the highest number of deaths occurred in the 70-74 age group, while the 65-69 group had the largest DALY burden in 2021. Notably, females showed a disproportionately greater increase in burden than males, with deaths increasing by 238% versus 136%, and DALYs by 203% versus 109%.<br /><br />In 2021, HFPG-attributable TBL cancers accounted for 2.51% of all such cancer deaths. The findings highlight the growing intersection between metabolic dysfunction—driven by rising HFPG due to urbanization and lifestyle shifts—and lung cancer risk among G20 nations. The study underscores an urgent need for integrated public health policies, clinical management, and preventive strategies targeting both metabolic health and cancer prevention to mitigate this rising health challenge globally.
