PT1.04.03 The LEAP Study: A Biospecimen Bank for Validation of Liquid Biomarkers in a Lung Cancer Screening Cohort

The LEAP Study is a multicenter, six-year lung cancer screening project focused on creating a biospecimen bank to validate liquid biomarkers in a screening cohort. Conducted by MD Anderson Cancer Center and collaborators, the study enrolled 2,846 participants across multiple U.S. and international centers. Biospecimens were collected longitudinally during baseline and annual screenings (Years 0-2) using standardized protocols, with an extensive aliquoting system centrally managed at MD Anderson. Participants also completed questionnaires from baseline through Year 5.<br /><br />The cohort includes participants from 10 investigational centers, such as MD Anderson, USC Norris Cancer Center, Ohio Health System, Stanford University, and international centers including Fundación MD Anderson International in Spain. Follow-up periods varied by site, averaging between approximately 2 to 4.6 years. The study maintained longitudinal collections of low-dose CT scans and blood biospecimens at baseline, Year 1, and Year 2, with biospecimen collection rates generally around 20-39% per center.<br /><br />Over three years of screening, 72 lung cancers were detected, with detailed staging available for 65 cases. Among these, most cancers were early-stage (36 stage I), while stages II-IV included fewer cases. Histological subtypes included 40 adenocarcinomas, 13 squamous cell carcinomas, and 12 small cell lung cancers. Additionally, 101 other non-lung cancers were diagnosed within the cohort, including prostate, urothelial, breast, colorectal, lymphoma, head and neck, and renal cell cancers.<br /><br />In summary, the LEAP Study has established a well-characterized biospecimen resource linked to clinical, radiologic, and questionnaire data from a large, diverse lung cancer screening population. This bank supports ongoing validation studies of blood-based biomarkers for early lung cancer detection and related malignancies.
