Researchers from various institutions in Spain conducted a retrospective study to analyze the prognostic factors and impact of silibinin supplementation on survival in patients with non-small cell lung cancer (NSCLC) who developed brain metastases (BM). The study found that silibinin, a bioactive flavonolignan extracted from milk thistle, may aid in the management of BM in NSCLC patients.<br /><br />The study included NSCLC patients diagnosed with BM in the province of Girona between 2010 and 2019. Electronic clinical records were reviewed to collect information about BM, patient, and tumor characteristics. The follow-up cut-off date was December 31, 2022.<br /><br />During the follow-up period, 457 out of 2,830 NSCLC patients developed BM. The median overall survival (OS) varied based on the histological subtype of NSCLC, with adenocarcinoma having the longest median OS at 4.44 months. Among the 211 patients with NSCLC and BM who received brain radiotherapy and systemic treatment, 57 received silibinin.<br /><br />The analysis showed significant differences in OS between patients who received silibinin treatment and those who did not, with the silibinin group having a median OS of 10.6 months compared to 6.3 months in the non-silibinin group. A multivariate analysis identified silibinin treatment as a statistically significant independent variable associated with improved survival.<br /><br />These findings suggest that silibinin supplementation may have a positive impact on survival in NSCLC patients with BM. Prospective clinical trials evaluating silibinin or other STAT3 inhibitors are needed to further validate these results. The study was supported by Fundació La Marató de TV3.
