EP03.05. Genomic Landscape Features of Minimally Invasive Adenocarcinoma and Invasive Lung Adenocarcinoma - PDF(Slides)

A study was conducted to investigate the genomic features between minimally invasive adenocarcinoma (MIA) and invasive adenocarcinoma (IAC) in patients with lung adenocarcinoma. The researchers retrospectively analyzed genetic data from 119 MIA patients and 303 IAC patients using targeted multigene panel sequencing.<br /><br />The results showed that MIA patients had a higher frequency of mutations in EGFR, ERBB2, KRAS, BRAF, MET, TP53, BRAC2, RET, and PIK3CA genes, compared to IAC patients. On the other hand, IAC patients had a higher frequency of mutations in EGFR, TP53, KRAS, PIK3CA, CDKN2A, ERBB2, and CDK4 genes.<br /><br />Interestingly, gene fusions were identified in the MIA cohort, with all cases being RET fusion cases. However, RET fusions were less common in the IAC cohort. Nine IAC patients were found to carry germline variants, including WRN, PALB2, MRE11A, BRCA1, RAD51D, BRCA2, CHEK2, and ATM, which were not detectable in MIA patients.<br /><br />The study also revealed that mutations in ERBB2, BRAF, and RET fusion are early clonal genomic events in the carcinogenesis of lung adenocarcinoma, while TP53, CDKN2A, and metastasis-related genes may be late events. Genomic intratumor heterogeneity was observed as a common and early phenomenon.<br /><br />In terms of patient characteristics, MIA patients were more likely to be female and younger, compared to IAC patients.<br /><br />Overall, the study provides insights into the genomic landscape features of MIA and IAC in lung adenocarcinoma patients, highlighting the differences in mutation frequencies and gene fusions between the two groups. These findings contribute to a better understanding of the molecular mechanisms involved in the development and progression of these subtypes of lung adenocarcinoma.
