Pdf Summary
This study aimed to investigate the molecular epidemiology and clinical outcomes of KRAS-mutant non-small cell lung cancer (NSCLC) in Brazil. The study included 533 patients with NSCLC, of whom 140 had KRAS mutations, accounting for 26.2% of the population. The most common KRAS mutation was G12C, followed by G12D. The frequency of co-mutations in TP53, STK11, and KEAP1 genes was observed to be 30.7%, 24.2%, and 12.1%, respectively. PD-L1 expression was found to be 1% in 52.4% of cases.

In terms of treatment, the majority of patients received a combination of chemotherapy and immunotherapy in the first line, followed by chemotherapy alone or immunotherapy alone. Only three patients received targeted therapy against the KRAS mutation. The overall survival of KRAS-mutant NSCLC patients was found to be numerically worse compared to KRAS wild-type patients, but further follow-up is needed to make a formal comparison. The study highlights the need for more data on the molecular epidemiology of KRAS mutations and their impact on clinical outcomes.

The study provides real-world data from the largest private community oncology practice in Brazil, Oncoclinicas. It contributes to the understanding of KRAS-mutant NSCLC in the Brazilian population and sheds light on the prevalence of specific KRAS mutations and their co-mutations. The findings reinforce the importance of targeted therapies for KRAS-mutant NSCLC and the potential to improve clinical outcomes for these patients.
