This study evaluated two potential criteria for enrolling participants into a lung cancer screening pilot project in Estonia. The criteria included smoking history and a lung cancer risk score. The screening project was conducted in one county of Estonia over a one-year period. Participants were enrolled by family physicians and nurses, and those with elevated lung cancer risk according to either the smoking history criterion or the risk score were assigned low-dose computed tomography (LDCT) scans. The scans were evaluated using the LungRADS version 1.1 and patients were managed based on the results.<br /><br />A total of 26,759 patients met one or both inclusion criteria and were referred for LDCT scans. Out of the eligible patients, 92.9% underwent LDCT scans and 28 lung cancer cases were confirmed. Based on smoking status, 3499 patients (14.3% of the target age group) had elevated lung cancer risk and underwent LDCT scans, with 27 new lung cancer cases detected. Based on the risk score, 2496 patients (10.2% of the target age group) had elevated risk and underwent LDCT scans, with 25 new lung cancer cases detected.<br /><br />According to the findings, enrolling participants based on either smoking status or the risk score was equally feasible for family physicians and nurses. However, using the risk score as an inclusion criterion would have resulted in a significant reduction in the number of LDCT scans. There was no significant difference in cancer detection rate between the two criteria, likely due to the small number of lung cancer cases. Overall, this study provides insights into the implementation difficulties and results of different enrolment criteria in a lung cancer screening pilot project in Estonia.
