EP04.06. Compliance with Lung Cancer Screening Guidelines Among At-Risk Patients at a Rural Teaching Hospital in West Texas - PDF(Slides)

This study aimed to determine the compliance with lung cancer screening guidelines among at-risk patients at a rural teaching hospital in West Texas. The researchers conducted a retrospective cohort study of patients in the University Medical Center health system from 2018 to 2022. The study included patients aged 50 to 80 who met the USPSTF lung cancer screening guidelines and underwent an initial low-dose CT scan. The primary outcome was subsequent lung cancer screening within the recommended timeframe based on the initial lung-RADS score.<br /><br />The study included 561 patients, the majority of whom were male, white, and non-Hispanic. At the initial screening, the majority of patients had a lung-RADS score of 2 or lower. Only 33.8% of patients with a lung-RADS score of 3 received a second scan within 6 months. More than half of the initial sample were lost to follow-up according to the screening guidelines.<br /><br />There was no statistically significant difference in follow-up rates based on race or ethnicity. However, there was a significant difference in follow-up rates based on gender, with a higher percentage of males being lost to follow-up compared to females.<br /><br />The researchers concluded that there is a need for improvement in retaining patients in the screening pipeline. Although there was no serious disparity in follow-up among racial and ethnic minorities, future work should focus on encouraging all patients, especially males, to follow screening guidelines. The study suggests the need to identify barriers to follow-up, increase patient education, and improve outreach to improve lung cancer screening adherence.
