EP06.02. Tertiary Lymphoid Structures Predict Clinical Outcomes of Neoadjuvant Chemoimmunotherapy in Resectable NSCLC - PDF(Abstract)

This study investigated the prognostic value of tumor-associated tertiary lymphoid structures (TLS) in patients with resectable non-small cell lung cancer (NSCLC) receiving preoperative neoadjuvant chemoimmunotherapy. The researchers retrospectively analyzed 98 patients from two cohorts: neoadjuvant chemoimmunotherapy (N57) and neoadjuvant chemotherapy (N41). TLSs were identified and quantified using H&E staining, and a scoring system was established for inside tumor bed region (IT score) and outside tumor bed region (OT score). The researchers also used multiplex immunohistochemistry (mIHC) to characterize TLS maturation. <br /><br />The results showed that the density of TLS was higher in the neoadjuvant chemoimmunotherapy cohort compared to the neoadjuvant chemotherapy cohort. Patients with higher TLS density inside the tumor bed region had a more favorable prognosis in both cohorts. TLS density outside the tumor bed region did not show a significant correlation with clinical outcomes. Additionally, the analysis of TLS maturation showed that both primary and mature TLS were higher in the neoadjuvant chemoimmunotherapy cohort. TLS maturation was found to be an independent predictor for recurrence-free survival (RFS) in the neoadjuvant chemoimmunotherapy cohort.<br /><br />In conclusion, the density, spatial distribution, and maturation of tertiary lymphoid structures are significantly associated with clinical outcomes in resectable NSCLC with neoadjuvant chemoimmunotherapy. These findings suggest that TLS could serve as a valuable predictive factor for RFS in these patients.
