EP06.03. Molecular Alterations in Non-Small Cell Lung Cancer Arising in Emphysematous and Fibrotic Lungs - PDF(Slides)

This study examined the molecular alterations in non-small cell lung cancer (NSCLC) that arises in emphysematous and fibrotic lungs. The researchers analyzed consecutive lung cancer resection specimens from patients with NSCLC and histologic evidence of smoking-related centrilobular emphysema (CLE) and usual interstitial pneumonia (UIP)/idiopathic pulmonary fibrosis (IPF). They reviewed the results of targeted next-generation sequencing (NGS) of the resected NSCLC samples.<br /><br />The findings showed that NSCLC in emphysematous lungs had different molecular alterations compared to those in fibrotic lungs. This suggests that the tissue environment of chronic lung injuries may contribute to tumorigenesis in different ways.<br /><br />Chronic obstructive pulmonary disease (COPD) and idiopathic pulmonary fibrosis (IPF) are both chronic lung diseases with similar risk factors, and each is independently associated with an increased risk of lung cancer. However, the contribution of lung parenchymal remodeling in emphysema or fibrosis to tumorigenesis is not fully understood.<br /><br />The results of the study revealed that KRAS mutations were the most frequent alteration in NSCLC arising in emphysematous lungs, while TP53 mutations were the most common in NSCLC arising in fibrotic lungs. This suggests that different molecular mechanisms may be involved in the development of NSCLC in these two lung conditions.<br /><br />In conclusion, this study highlights the importance of considering the tissue environment in understanding the molecular alterations and tumorigenesis of NSCLC in patients with emphysematous and fibrotic lungs. Further research is needed to better comprehend the underlying mechanisms and potential therapeutic targets for these distinct lung conditions.
