EP06.05. Distant Metastasis Pattern and Baseline Clinicopathological Features from a Real-World KRAS G12C Mutant Cohort of Advanced NSCLC Patients - PDF(Abstract)

This study aimed to investigate the clinicopathological features and metastasis patterns of advanced non-small cell lung cancer (NSCLC) patients with KRAS G12C mutation. The researchers conducted a retrospective analysis of newly diagnosed patients with stage IIIC-IV NSCLC who underwent next-generation sequencing (NGS) testing between September 2022 and March 2023. A total of 62 patients were included in the study, with adenocarcinoma being the most frequent histology (90.3%). The results showed that 38.7% of patients had KRAS mutations, with the most common mutation being G12C (37.5%).<br /><br />The researchers found that KRAS mutant patients were more frequently current/former smokers compared to KRAS wild-type patients. However, there was no significant difference in PD-L1 expression levels between the two groups. No concomitant oncogenic driver mutations were observed in the KRAS mutant cohort.<br /><br />In terms of metastasis patterns, the study found that G12C mutant patients had a higher incidence of brain metastases compared to non-G12C patients (66.6% vs 13.3%). They also had a greater number of metastasis sites at the time of diagnosis, particularly in the brain. However, there was no significant difference in liver metastasis distribution between the two groups.<br /><br />Overall, the findings of this study suggest that KRAS mutant patients with NSCLC represent a highly heterogeneous population. The G12C subgroup, in particular, showed a distinct metastasis pattern with a higher incidence of brain metastases. These results highlight the need for better characterization of KRAS mutant patients in order to tailor treatment strategies.
