EP06.05. Non Small Cell Lung Cancer Associated to Dipnech: A Clinico-Pathological Analysis of 32 Cases - PDF(Abstract)

This study provides a clinical and pathological analysis of 32 cases of non-small cell lung cancer (NSCLC) associated with Diffuse Idiopathic Pulmonary Neuroendocrine Cell Hyperplasia (DIPNECH). DIPNECH is a condition that has been suggested to be a precursor to lung carcinoids, but there is limited data on its association with NSCLC. The authors retrospectively analyzed their institutional database of surgically-treated NSCLC patients and identified 32 cases (2.5%) with an incidental diagnosis of DIPNECH. They collected clinical, functional, radiological, and histological data and analyzed long-term results.<br /><br />The study found that the majority of patients with NSCLC associated with DIPNECH were female (26/81%) with a mean age of 70 years. Lung function showed an obstructive pattern with irreversible airflow obstruction in 37.5% of patients, but only 3 were active smokers. Symptoms such as non-productive cough and wheezing were observed in 8 patients. CT scans revealed bilateral multiple pulmonary nodules, air-trapping, pulmonary emphysema, mucus plugging, pulmonary fibrosis, and bronchiectasis in some patients.<br /><br />Surgery was the primary treatment approach, with lobectomy being the most common procedure. Pathological analysis showed that the most frequent histology was adenocarcinoma, followed by squamous cell carcinoma and large-cell carcinoma. Some cases also showed coexistence with DIPNECH and typical carcinoid. Positive staining for neuroendocrine markers was observed in a subset of cases.<br /><br />Overall, the study suggests that DIPNECH may be associated with a low but significant percentage of NSCLC cases, particularly adenocarcinomas. Despite the small number of cases, patients with NSCLC associated with DIPNECH had a favorable prognosis and some specific clinical manifestations. The authors highlight the need for further research to investigate this topic due to the lack of available data.
