EP06.05. Non Small Cell Lung Cancer Associated to Dipnech: A Clinico-Pathological Analysis of 32 Cases - PDF(Slides)

This study aimed to investigate the incidence and clinico-pathological features of patients who underwent surgery for non-small cell lung cancer (NSCLC) concomitant with diffuse idiopathic pulmonary neuroendocrine cell hyperplasia (DIPNECH). DIPNECH is a rare condition characterized by the proliferation of neuroendocrine cells in the lung. The study included 32 patients out of 1284 surgically-treated NSCLC patients (2.5%) who had an incidental diagnosis of DIPNECH in the lung specimens.<br /><br />The majority of the patients were female (81%) with a mean age of 70 years. Lung function showed an obstructive pattern with irreversible airflow obstruction in some patients. The CT scans revealed bilateral multiple pulmonary nodules in 40.6% of the patients, as well as other radiological signs such as air-trapping, pulmonary emphysema, mucus plugging, and signs of pulmonary fibrosis. The surgical procedures performed included mostly lobectomy (59.4%) and sub-lobar resection (34.4%) with no post-operative mortality or major complications.<br /><br />The most frequent histology observed was adenocarcinoma (3.8% of all adenocarcinomas operated on), followed by squamous cell carcinoma and large-cell carcinoma. Some adenocarcinomas were associated with DIPNECH and typical carcinoid. Immunohistochemical markers such as Synaptophysin, Chromogranin A, and TTF1 were positive in a subset of cases.<br /><br />The patients had a good prognosis, with only 9.4% experiencing local recurrence after 4 years. Overall, DIPNECH was found in a small but significant percentage of NSCLC cases, particularly adenocarcinoma. The researchers recommend further investigation into this topic due to the limited number of cases and the unique clinical manifestations observed in these patients. Therefore, DIPNECH should be considered in the differential diagnosis of NSCLC and further research is needed to better understand its implications in clinical practice.
