A study was conducted to investigate the impact of an intraoperative needle biopsy (INB) on the recurrence of preoperatively undiagnosed stage 1 lung cancer. The study included 240 cases of radical lobectomy for lung cancer from January 2012 to December 2017. A 22- to 23-G needle biopsy was performed on a subset of patients, while others did not undergo the biopsy. Propensity score matching was used to compare the outcomes between the two groups.<br /><br />The diagnostic rate of the INB with a 22- to 23-G needle was 89.0%, and no INB-related complications were observed. The recurrence rates, site of recurrence, and 5-year relapse-free survival rates were similar between the INB group and the non-INB group. Cox proportional hazards regression analysis showed that pleural invasion, lymphatic invasion, and vascular invasion were associated with recurrence, but performing an INB did not increase the risk of recurrence.<br /><br />Based on these findings, the study concluded that an INB with a 22- to 23-G needle is a safe and useful technique for preoperatively undiagnosed stage 1 lung cancer. The INB did not affect the recurrence rates, suggesting that this biopsy method does not increase the risk of cancer recurrence.<br /><br />This study provides evidence supporting the use of INB as a diagnostic tool for lung cancer. It helps determine the appropriate surgical approach for lung resection surgery. The study also addresses concerns about the potential risks associated with INB, such as dissemination or recurrence of the cancer. Overall, this research contributes to the best practices for managing early-stage non-small cell lung cancer.
