EP07.01. Definitive Chemoradiation versus Radiation Alone for Stage II Non-Small Cell Lung Cancer. A National Cancer Database Analysis - PDF(Abstract)

A retrospective analysis of the National Cancer Database (NCDB) was performed to compare outcomes in patients with stage II Non-Small Cell Lung Cancer (NSCLC) treated with either definitive chemoradiation or radiation alone. The study aimed to determine the optimal treatment for these patients as there is limited data available.<br /><br />The study included 4650 patients diagnosed with stage II NSCLC between 2004 and 2018. They were divided into three treatment groups: radiation only, chemotherapy only, and chemoradiation. The median age at diagnosis was 71, and the majority of patients were non-Hispanic, white, male, with Medicare coverage, and had squamous cell carcinoma.<br /><br />The five-year survival rates were 13.2% for radiation alone, 16% for chemotherapy alone, and 24.9% for chemoradiation. The addition of chemotherapy to definitive radiation significantly improved overall survival. Multivariate analysis showed that the hazard ratio for chemoradiation versus radiation only was 0.75, indicating a lower risk of death with chemoradiation.<br /><br />Subgroup analysis found that the treatment benefit from chemoradiation did not vary based on age, gender, ethnicity, histology, or co-morbid status. This suggests that adding chemotherapy to radiation is beneficial for all patients with stage II NSCLC, regardless of individual characteristics.<br /><br />In conclusion, this study found that patients with stage II NSCLC who cannot undergo surgery predominantly have squamous cell histology. The addition of chemotherapy to definitive radiation improves overall survival in these patients. These findings provide valuable information for guiding treatment decisions for early-stage NSCLC patients.
