EP07.01. Factors Affecting Postoperative Nausea and Vomiting in Surgical Patients with Non-Small Cell Lung Cancer - PDF(Abstract)

This study aims to investigate factors affecting postoperative nausea and vomiting (PONV) in patients with non-small cell lung cancer (NSCLC) who undergo surgery. PONV is known to be distressing for surgical patients, and this study aims to fill the gap in the limited reports on PONV in the field of thoracic surgery. The study included 256 patients who underwent anatomical resection for NSCLC between October 2021 and September 2022.<br /><br />The researchers assessed the differences in clinical characteristics and perioperative factors between patients with and without PONV, including the use of Granisetron, Droperidol, and dexamethasone. PONV was evaluated based on three grades: vomiting or nausea (G1), vomiting or severe nausea affecting food intake (G2), and vomiting or antiemetic use (G3). The occurrence rate of PONV was also investigated according to the Apfel score.<br /><br />The results showed that Apfel score 2 was strongly related to PONV in thoracic surgery. Droperidol use was found to be effective in reducing PONV in patients with thoracic surgery, but not enough to reduce the need for antiemetic rescues. Granisetron showed a trend toward preventing PONV but did not have a significant effect in this study. The study suggests that in thoracic surgery, the prophylactic administration of multiple antiemetic agents, including 5-HT3 receptor antagonists and D2 receptor antagonists, should be considered for patients with an Apfel score of 2. Further prospective studies are needed to explore this approach.<br /><br />In conclusion, this study identified factors affecting PONV in patients with NSCLC undergoing surgery and highlighted the importance of individualized prophylactic antiemetic strategies based on Apfel score in thoracic surgery.
