EP07.01. Long Term Outcomes beyond Five Years after Pulmonary Resection for Non Small Cell Lung Cancer - PDF(Abstract)

This study aimed to investigate the long-term outcomes of non-small cell lung cancer (NSCLC) patients who underwent pulmonary resection and survived five years without recurrence. The researchers retrospectively reviewed medical records of 978 NSCLC patients who underwent lobectomy or more extended resection between 2002 and 2015. They found that 3.8% of patients experienced postoperative late recurrence (LR) beyond five years. Factors associated with LR included high preoperative serum CEA level, adenocarcinoma histology, pN1-2, pStage II-III disease, and presence of adjuvant chemotherapy. Adenocarcinoma patients with pN1-2 had significantly poorer long-term recurrent free survival (RFS) rates. Meanwhile, those with pN0 and non-adenocarcinoma histology had favorable RFS beyond five years after surgery.<br /><br />The study also investigated the incidence of late metachronous malignancies, including lung cancer, in these patients. They found that 11.8% of patients experienced late metachronous malignancies, with 5.8% developing lung cancer. Other late metachronous malignancies included gastric cancer and colorectal cancer.<br /><br />Based on these findings, the researchers concluded that non-adenocarcinoma histology and pN0 status are significant favorable factors for long-term RFS after lung cancer resection. However, the efficacy of long-term surveillance for detecting LR seems to be limited for these patient populations. They suggest that early detection of late metachronous malignancies, especially lung cancer, may lead to improved outcomes.<br /><br />In summary, this study provides insights into the long-term outcomes and factors associated with LR and late metachronous malignancies in NSCLC patients who underwent pulmonary resection. The findings highlight the importance of considering histology and pN status in determining long-term RFS and the need for early detection of late metachronous malignancies for improved patient outcomes.
