EP07.01. Perioperative Outcomes & Strategies for Pleomorphic Carcinoma in Lung - PDF(Abstract)

This presentation discusses the perioperative outcomes and strategies for pleomorphic carcinoma in the lung, a rare subtype of lung cancer. Pleomorphic carcinoma represents only 0.3% of invasive non-small cell lung cancer cases and is typically found more frequently in the right lung and upper lobe. The prognosis for this type of lung cancer is generally worse than other types of non-small cell lung cancer.<br /><br />The study retrospectively analyzed clinic-pathological features of 10 patients who underwent lobectomy for pleomorphic carcinoma from September 2012 to February 2023. The patients had a median age of 76 years, with a male to female ratio of 7:3. Preoperative histology showed that 5 patients had adenocarcinoma. The tumors were mostly located in the right upper lobe, and the median size of the tumor was 39.5 mm on preoperative CT images.<br /><br />The surgical procedures resulted in 4 complications, including respiratory failure, infection in the reconstructed chest wall, and arrhythmia. The median hospital stay was 8 days, and the median length of chest tube drainage was 4 days. Pathological staging revealed various stages, with vascular invasion identified in all specimens. Three patients received adjuvant chemotherapy, while 7 patients experienced disease relapse.<br /><br />The study highlights the devastating progression of pleomorphic carcinoma and emphasizes individualized approaches for each patient. Immediate evaluation of genetic mutations, such as EGFR mutation, and PD-L1 expression status in resected specimens on diagnosis, as well as shorter follow-up intervals for imaging to detect recurrence and start prompt treatment, are recommended.<br /><br />In conclusion, this study sheds light on the challenges posed by pleomorphic carcinoma and emphasizes the need for personalized treatment strategies to improve outcomes in patients with this rare subtype of lung cancer.
