Lung cancer is a common and deadly form of cancer worldwide, with recurrence after surgery being a major contributor to decreased life expectancy and mortality. However, the patterns of recurrence, prognostic factors, and their impact on survival have not been clearly defined. In this study, the researchers aimed to identify recurrence patterns and prognostic factors in patients with completely resected non-small cell lung cancer (NSCLC). The long-term results of patients who underwent surgery for lung cancer between 2010 and 2015 were retrospectively evaluated. Patients with small cell lung cancer, carcinoid tumor, metastatic lung cancer, incomplete resection, or previous lung cancer diagnosis were excluded from the study. A total of 783 patients were included, and during the follow-up period, recurrences were observed in 41.6% of cases. Loco-regional recurrence was the most common pattern, followed by systemic recurrence. The most common sites of local recurrence were the anastomosis line in the ipsilateral lung and the hilar-mediastinal lymph nodes. The most common areas of systemic recurrence were the brain, contralateral lung, bone, liver, and adrenal glands. Isolated distant metastasis, most frequently in the brain, was observed in 2% of cases. Brain metastasis was associated with a significantly higher risk of death. The TNM stage of the cancer was also found to be correlated with the probability of recurrence. Overall, the study provides insight into the recurrence patterns and prognostic factors for relapse in operated lung cancers, which may contribute to improved management and survival outcomes for NSCLC patients.
