EP07.01. Surgical Treatment of Lung Cancer in Patients Over the Age of 75 Years: Single Cancer Centre Experience - PDF(Slides)

This study focused on the surgical treatment of lung cancer in patients over the age of 75 at the Moscow Oncology Research Institute of P.A. Herzen in Russia. The study highlighted the fact that the global population is aging, and by 2050, over 20% of the population will be over the age of 65. It was found that lung cancer is more frequently diagnosed in patients over 70 years old. However, elderly patients are often denied surgical treatment due to comorbidities or perceived risk. <br /><br />The study analyzed the data of 73 patients aged 75 years and older who underwent surgery for lung cancer. The majority of the patients were males (54.8%), and the most common type of lung cancer was adenocarcinoma (60.3%). The study also looked at the resection type performed, with lobectomy being the most common (68.5%). Complications were observed in 16.4% of the cases, with atrial fibrillation being the most common. <br /><br />The study found that there were fewer complications in octogenarians compared to younger patients. It also observed that the incidence of complications varied depending on surgical access, with thoracotomy resulting in a higher incidence of complications compared to video-assisted thoracoscopic surgery (VATS). Lower stage tumors, lobectomy, and adenocarcinoma were associated with a lower risk of complications. <br /><br />The study concluded that age itself should not be a contraindication to surgical treatment for lung cancer in elderly patients. However, a thorough assessment of risk factors and careful surgical planning should be done due to the elevated mortality rate. The involvement of cardiologists and anesthesiologists in the preoperative preparation was also deemed important. Favorable factors for surgical treatment included peripheral adenocarcinoma, stage I tumors, thoracoscopic approach, N0 lymph node status, and a Charlson index of less than 6.
