This study examines the impact of comorbidities on exercise levels in patients treated for early-stage lung cancer. The researchers used a prospective cohort of stage I non-small cell lung cancer patients who underwent either surgery or stereotactic body radiation therapy (SBRT). Exercise levels were measured using a questionnaire at baseline and at 0-4, 4-8, and 8-12 months after treatment. Patients were stratified based on the presence or absence of comorbidities such as chronic obstructive pulmonary disease (COPD), diabetes, cardiac disease, and vascular disease, as well as treatment options (surgery vs. SBRT). <br /><br />The study included 417 patients, with 62% undergoing SBRT and 85% undergoing surgery. Patients who underwent SBRT had lower exercise levels at baseline and during all subsequent time points compared to surgical patients. Patients with COPD had lower exercise levels at baseline compared to those without COPD. However, at the end of the first year after treatment, patients with COPD showed an increase in exercise levels, although it was not statistically significant. Patients without COPD experienced a decline in exercise levels during the first 4 months after treatment but recovered to their baseline levels by the end of the first year. Similar patterns were observed when analyzing surgical patients only.<br /><br />The results suggest that comorbidities can impact the trajectory of exercise levels after treatment for early-stage lung cancer. Surgical patients with comorbidities may benefit from exercise interventions to improve their recovery. Future research should investigate the long-term effects of such interventions and consider comorbidities when developing exercise plans for lung cancer patients.
