EP07.02. Localization of Lung Nodules by Electromagnetic Navigation Bronchoscopy-Guided Dye Marking: Procedural Outcomes and Learning Curve - PDF(Abstract)

This study focuses on the use of electromagnetic navigation bronchoscopy (ENB) for localizing lung nodules in minimally invasive thoracoscopic surgery. The researchers aimed to investigate the procedural outcomes and learning curve associated with ENB-guided dye marking.<br /><br />A total of 89 patients who underwent ENB-guided dye marking for small pulmonary nodules were included in the study. The mean nodule size was 9.0 mm, and the mean distance from the pleural surface was 8.1 mm.<br /><br />Overall, the success rate of nodule localization was 94.8%, with 108 out of 114 nodules successfully localized. Six failures were observed, involving specific lung segments. The researchers found that the duration of ENB decreased over time, indicating a learning curve with improved efficiency.<br /><br />The most challenging cases were observed in the right upper lobe apical segment, with 71.4% of cases considered difficult. Other segments with a higher proportion of difficult cases included the left upper lobe apicoposterior segment, the right lower lobe superior segment, the right lower lobe basal segment, and the left lower lobe basal segment.<br /><br />No complications related to ENB were reported in the study.<br /><br />In conclusion, ENB allows for minimally invasive resection of small pulmonary nodules with safety and accuracy. The study suggests that although certain lung segments may require more time and effort for localization, the overall failure rate is low. The researchers also suggest that a continual learning curve is to be expected, and additional assistance such as fluoroscopy or con-beam CT may be considered to increase accuracy and overcome the learning curve.
