This study examines the challenges in performing robotic lobectomy of the lung through the anterior approach. Robot-assisted thoracic surgery (RATS) has become popular due to its minimally invasive nature, but limited visibility and lack of tactile feedback can lead to complications. The authors adopted an anterior approach to overcome these challenges and perform RATS safely. The study included 35 patients who underwent RATS lobectomy between November 2020 and March 2023. The da Vinci Si robot was used, and ports were placed horizontally along the trunk's longitudinal axis. CO2 insufflation and stapling device operation were performed through an assist port. The results showed no severe incidences of vascular injury with the anterior approach, but complications related to the use of stapling devices were noted. Postoperative complications included pulmonary fistula, middle lobe torsion, and ventricular arrhythmia. There were no complications related to vascular injury, and no conversions to VATS/open procedures were needed. There were no significant correlations between clinical background factors, intraoperative blood loss, or the number of staplers used and postoperative complications, except for a significant correlation between console time and the number of staplers used. The authors conclude that while the anterior approach is safe in terms of vascular injury, careful consideration of the indications for RATS and stapler usage is necessary.
