EP07.03. Effect of Lymph Node Dissection on Recurrence and Survival Following Thoracoscopic Segmentectomy for Early-Stage Non-small Cell Lung Cancer - PDF(Abstract)

This study aimed to determine the effect of lymph node (LN) dissection in video-assisted thoracoscopic surgery (VATS) segmentectomy on the recurrence and survival of patients with early-stage non-small cell lung cancer (NSCLC). The researchers retrospectively analyzed data from 158 consecutive VATS segmentectomies performed between 2008 and 2022. The median number of LNs dissected was 9. Patients were followed up for a mean of 3.3 years. <br /><br />Results showed that 33 patients (20.9%) experienced recurrence. Pleural invasion, advanced stage, and upstaging to N1 and N2 were significantly associated with recurrence. Resection on the left lower lobe and upstaging to N2 were independent risk factors for recurrence-free survival (RFS), while ECOG status and upstaging to N2 were associated with overall survival (OS). <br /><br />Interestingly, the number of LNs dissected did not have a significant impact on the rates of overall, locoregional, or distant recurrence, as well as RFS and OS. <br /><br />Based on these findings, the authors concluded that the number of LNs dissected does not affect the recurrence rate or survival outcomes following thoracoscopic segmentectomy for early-stage NSCLC. This suggests that the extent of LN dissection may not be a critical factor in determining the prognosis of patients undergoing this procedure. <br /><br />Overall, this study contributes to the ongoing debate surrounding the optimal extent of LN dissection in early-stage NSCLC and provides insights into the factors influencing recurrence and survival outcomes in patients undergoing thoracoscopic segmentectomy.
