EP07.03. Impact of Tumor Location on Surgical Margin in Segmentectomy and Wedge Resection - PDF(Abstract)

This study aimed to investigate the impact of tumor location on surgical margin in segmentectomy and wedge resection for early-stage non-small cell lung cancer. A total of 618 patients who underwent sublobar resection were included in the study. The tumor location was classified into six categories using the Lewis classification. The surgical margin was examined by pathologists, and an adequate surgical margin was defined as 2 cm or the total tumor size. <br /><br />The results showed that segmentectomy achieved a higher proportion of cases with an adequate surgical margin compared to wedge resection. The median surgical margin was 2.2 cm in segmentectomy and 1.6 cm in wedge resection. Adequate surgical margin was achieved in 73% of segmentectomy cases and 64% of wedge resection cases.<br /><br />Furthermore, the tumor location was categorized as easy to resect or difficult to resect. In the easy-to-resect locations, the proportion of cases with an adequate surgical margin was significantly lower in wedge resection compared to segmentectomy. However, in the difficult-to-resect locations, the proportion with an adequate surgical margin was similar between wedge resection and segmentectomy.<br /><br />Based on these findings, the study concludes that segmentectomy is preferable to wedge resection for achieving an adequate surgical margin. The tumor location, as classified by the Lewis classification, should be taken into account when planning sublobar resection. Even for tumors where wedge resection can be easily performed, the proportion of cases with an adequate surgical margin after wedge resection is modest but unsatisfactory. Therefore, segmentectomy should be considered as the preferred surgical approach.
