Pdf Summary
A study presented at the World Conference on Lung Cancer focused on the use of uniportal video-assisted thoracoscopic surgery (u-VATS) for lobectomy in early-stage lung cancer. The study aimed to assess the nodal upstaging rate in u-VATS and compare it with other minimally invasive techniques. The authors conducted a retrospective analysis of patients who underwent u-VATS lobectomy for early-stage lung cancer at a tertiary center between March 2017 and December 2021. The primary endpoint was nodal upstaging, and secondary endpoints included operative time, chest-tube removal, morbidity and mortality rates, number of harvested nodes, and nodal recurrence rate.

The study included 160 patients with clinically early-stage lung cancer out of 221 who underwent u-VATS lobectomy. The mean operative time was 113.6 minutes, and the mean chest-tube duration was 3.4 days. The 30-day morbidity and mortality rates were 13.1% and 0%, respectively. The mean number of sampled nodes was 7.6, with a rate of nodal upstaging of 14.4%. Of the upstaged cases, 8.7% went from N0 to N1 and 5.6% went from N0 to N2. The 1-year nodal recurrence rate was 5%.

The authors concluded that u-VATS performed in experienced tertiary centers provides satisfactory node dissection with low morbidity and mortality rates. They found that the nodal upstaging rate in u-VATS was comparable to other minimally invasive approaches reported in the literature. The study highlighted the value of u-VATS as a safe and effective approach in surgical oncology, emphasizing the importance of thorough preoperative nodal evaluation.

The study also provided descriptive statistics on the patient cohort, including demographics, comorbidities, tumor characteristics, and surgical details. The authors used SPSS-23.0 software for statistical analysis.
