EP07.04. Clinical and Molecular Profile and Risk Factors of Stage IA NSCLC Relapsing after Radical Resection - PDF(Abstract)

This study aimed to identify the risk factors, clinical and molecular profile of relapsing radically resected stage IA non-small cell lung cancer (NSCLC). The researchers analyzed data from 313 patients who underwent R0 resection between January 2017 and December 2021. The majority of patients were female and the most common pathological stage was IA2. The recurrence rate was 9.3%, with loco-regional and distant recurrences being the most common.<br /><br />The median disease-free survival (DFS) in the overall cohort was 94 months, while in relapsing patients, it was 21 months. DFS was not influenced by factors such as gender, age, histology, anatomical resection, open surgery, tumor size, lymphovascular invasion, and EGFR status. However, smoking history, tumor grade, and ALK rearrangement were found to significantly influence DFS. ALK rearrangement was identified as the sole prognostic factor influencing DFS.<br /><br />In terms of overall survival (OS) in relapsing patients, the median OS was 76 months. Minimally invasive surgery to treat the primary cancer, a local recurrence, surgical treatment for the recurrence, and a major response after treatment were associated with better OS. The time to recurrence did not influence OS.<br /><br />Overall, the study highlights the possibility of recurrence in radically-resected stage IA NSCLC and identifies high-grade cancers and ALK rearrangement as risk factors for recurrence. The findings suggest the need for dedicated follow-up paths for these patients. Additionally, the study indicates that certain factors, such as minimally invasive surgery and a major treatment response, can improve OS in patients with recurrence.
