This study aimed to compare the clinical presentations and survival outcomes between non-small cell lung cancer (NSCLC) patients with occult lymph node metastasis (OLNM) and those with evident lymph node metastasis (ELNM). The researchers also sought to identify the predictive factors for OLNM. <br /><br />The study included a total of 2,067 eligible cases, with 21.4% of patients found to have OLNM. The analysis showed that patients with OLNM tended to be female, non-smokers, and have smaller-sized tumors compared to those with ELNM. <br /><br />Survival curves indicated that the survival outcomes of patients with OLNM were similar to those with ELNM, both before and after using propensity score matching to reduce bias. Multivariable Cox analysis revealed that positive lymph nodes (PLN) was the only prognostic factor for OLNM. <br /><br />Additionally, the random forest analysis identified clinical tumor size as an important predictive factor for OLNM. <br /><br />In conclusion, the study found that OLNM is not rare among resected NSCLC patients with lymph node metastasis and is not a favorable prognostic factor. The number of positive lymph nodes was found to determine the survival outcomes of patients with OLNM. Furthermore, clinical tumor size was a strong predictive factor for OLNM. <br /><br />The results of this study provide valuable insights into the prognosis and predictive factors associated with OLNM in NSCLC patients.
