EP07.04. Pathologic Risk Factors for Recurrence in Early Stage Invasive Lung Adenocarcinoma with Tumor Spread through Air Spaces - PDF(Abstract)

This study aimed to evaluate pathologic risk factors for recurrence in early stage lung adenocarcinoma with tumor spread through air spaces (STAS). The researchers reviewed the medical records of patients who underwent surgical resection for stage I lung adenocarcinomas between 2012 and 2021. Patients with invasive adenocarcinoma with a diameter of 2cm and presenting STAS were included in the study.<br /><br />Out of the 238 patients with invasive adenocarcinoma, 65 patients had STAS. The farthest distance of STAS from the tumor edge was found to be a significant risk factor for recurrence. However, the number of alveolar spaces did not show statistical significance in univariate analysis. The presence of a micropapillary pattern more than 10% was significantly higher in patients with a larger distance of STAS.<br /><br />In multivariate analysis, adjuvant therapy and STAS 2mm were identified as significant recurrence risk factors. The 5-year recurrence-free survival rates were 87.0% for patients in Group 1 (distance of STAS 2mm) and 49.6% for patients in Group 2 (distance of STAS > 2mm).<br /><br />Overall, the presence of STAS and the distance from the tumor edge were important risk factors for recurrence in early stage invasive lung adenocarcinoma treated with surgery. Additionally, the presence of a micropapillary pattern was associated with a larger distance of STAS.<br /><br />Keywords: lung adenocarcinoma, early stage, prognosis.
