EP07.05. Postoperative Recurrence Pattern and the Role of Adjuvant Therapy in Radiological Solid-Predominant Part-Solid Lung Adenocarcinoma - PDF(Abstract)

This study focused on the recurrence patterns and the role of adjuvant therapy in patients with solid-predominant lung adenocarcinoma. Solid-predominant nodules, which have a higher consolidation-to-tumor ratio compared to ground-glass opacity (GGO)-predominant nodules, are known to have a greater risk of recurrence and death. The goal of the study was to investigate the clinicopathological features of recurrent disease and the effectiveness of adjuvant therapy in these patients.<br /><br />The study included patients with clinical N0 solid-predominant nodules who underwent surgery between 2008 and 2015. The patients were followed for a median of 70 months, and their recurrence sites and adjuvant therapy data were recorded. Logistic regression analysis was used to identify predictors of recurrence, and survival outcomes were compared.<br /><br />Of the 462 eligible patients, 19% experienced postoperative recurrence. The recurrence patterns included locoregional-only, distant-only, and combined recurrences. The overall survival at 5 years varied depending on the recurrence patterns. Multivariate analysis identified lymph node involvement, lymphovascular invasion, and larger solid component diameter as independent risk factors for postoperative recurrence. Recurrent disease was also associated with epidermal growth factor receptor (EGFR) mutation and anaplastic lymphoma kinase (ALK) rearrangement.<br /><br />The use of tyrosine kinase inhibitors (TKIs) and late recurrence were identified as independent predictors of favorable outcomes after recurrence. TKIs were found to have significant advantages over chemotherapy in patients with recurrent disease. The study concluded that postoperative recurrence is not uncommon in patients with solid-predominant nodules and significantly impacts long-term survival. Patients with risk factors for recurrence should receive active follow-up and effective adjuvant therapy. The study also highlighted the importance of recurrence patterns and adjuvant treatment strategy in predicting survival after recurrence and emphasized the potential benefits of targeted therapy in patients with recurrent disease.
