EP08.02. Comprehensive Assessment of Pulmonary Function in Patients Receiving Radical (Chemo)Radiotherapy for Carcinoma Lung - PDF(Abstract)

This study conducted at the Tata Memorial Hospital in Mumbai aimed to assess the pulmonary function and radiation pneumonitis in patients receiving radical (chemo)radiotherapy for lung cancer. The researchers analyzed data from 135 patients who underwent definitive chemoradiotherapy using modern treatment planning systems. The incidence of grade 3 radiation pneumonitis was found to be 14% at 3 months and 4% at 6 months. Various factors such as lung volume receiving specific radiation doses, baseline forced vital capacity, and diffusing capacity of the lungs were identified as significant predictors of radiation pneumonitis. The study also observed a decline in diffusing capacity at 3 months with a minimal increase in subsequent follow-ups. Furthermore, 52.5% of patients developed grade 3 radiological pneumonitis, although this did not correlate significantly with clinical grading. The study found that patients with clinically significant radiation pneumonitis had poor physical and emotional functions. The researchers concluded that diffusing capacity is a sensitive parameter to assess radiation-induced lung injury and emphasized the need for a comprehensive model to predict the development of radiation pneumonitis that incorporates clinical tests, dosimetric parameters, radiological assessment, and changes in pulmonary function tests. In summary, this study provides valuable insights into the assessment of pulmonary function and the development of radiation pneumonitis in patients undergoing radical (chemo)radiotherapy for lung cancer.
