EP08.02. Comprehensive Assessment of Pulmonary Function in Patients Receiving Radical (Chemo)Radiotherapy for Carcinoma Lung - PDF(Slides)

A study conducted at Tata Memorial Hospital in India aimed to comprehensively assess the pulmonary function and radiation-induced lung injury (RILI) in patients receiving radical radiotherapy for lung cancer. The study included 135 patients with locally advanced lung cancer who underwent chemoradiation. The researchers evaluated the temporal evolution of radiation pneumonitis, changes in pulmonary function, thoracic imaging, and various clinico-dosimetric factors.<br /><br />The results showed that the incidence of Grade 3 radiation pneumonitis was 14% at 3 months and 4% at 6 months. Pulmonary function tests (PFTs) demonstrated a decline in DLCO at 3 months, followed by a minimal increase in subsequent follow-ups. The study also identified various factors that influenced changes in PFTs, including age, gender, COPD, smoking status, tumor characteristics, and treatment-related factors.<br /><br />Radiological assessment using the RGS scale showed that 52.5% of patients developed grade 3 radiological pneumonitis, but this did not have a significant correlation with clinical grading. The researchers concluded that DLCO is a sensitive parameter for assessing RILI, and a comprehensive functional grading or model is needed to predict the development of radiation pneumonitis, incorporating clinical tests, dosimetric parameters, radiological assessment, and changes in PFTs.<br /><br />Overall, this study provides valuable insights into the pulmonary function and radiation-induced lung injury in patients undergoing radical radiotherapy for lung cancer. The findings highlight the importance of considering various factors and comprehensive assessments to guide treatment and minimize toxicities in these patients.
