EP08.02. Dosimetric Comparison of Free- Breathing versus Respiratory Motion-managed Radiotherapy with 4D-CT Based VMAT in Lung Cancer - PDF(Abstract)

This study aimed to use respiratory motion-managed radiotherapy to reduce side effects in patients with non-small cell lung cancer and compare dosimetric factors with free-breathing planning. Simulation images were obtained in 10 respiratory phases using 4D-CT scanner, and target volumes were created for volumetric arc treatment. Thirty-one patients were evaluated, with a median age of 63 and a male-to-female ratio of 27:4. Objective response rates were 75% in the first month and 78% in the third month. Tumor volumes were similar, but planned target volumes were significantly lower. Dose coverage of tumor or target volumes was significantly higher in treatment plans, and risky organ doses were lower. Grade 1-2 radiation pneumonitis was seen in 35% of cases. Five cases had COVID-19 infection during or after treatment. Mean overall survival was 17.8 months, and the 2-year survival rate was 26%. In univariate analysis, objective response at the first month, maximum standardized uptake value at the third month, and 5 cycles of chemotherapy were significant for overall survival. In multivariate analysis, number of fractions, lung V30, and internal target volume were significant for overall survival. The study concluded that respiratory motion-managed radiotherapy with 4D-CT planning achieved accurate target coverage, lower risky organ doses, and a 2-year survival rate of 26% without serious side effects.
