This document is a summary of a retrospective study conducted on patients with clinical stage II-III non-small cell lung cancer (NSCLC) who underwent neoadjuvant immunotherapy (IT). The study aimed to determine if PET parameters could predict the pathological response to neoadjuvant IT in these patients.<br /><br />A total of 9 patients were included in the study, with 6 of them achieving a pathological complete response (pCR) after neoadjuvant IT. PET parameters, such as maxSUV, metabolic tumor volume (MTV), and total lesion glycolysis (TLG) were analyzed using pre-IT and post-IT PET/CT images. The study found that there were significant differences in these PET parameters between the pCR and non-pCR groups.<br /><br />Specifically, the pCR group had lower values for maxSUV, TLG, and MTV compared to the non-pCR group in both pre-IT and post-IT PET scans. The tumor to liver ratio (TLR) was also significantly different between the two groups.<br /><br />Two case examples were provided in the summary. One patient from the non-pCR group experienced tumor recurrence after surgery, while another patient from the pCR group remained recurrence-free during a 4-year follow-up period.<br /><br />In conclusion, the study suggests that FDG PET/CT could be a useful tool for predicting the pathological response to neoadjuvant IT in stage II-III NSCLC patients. The PET parameters analyzed in this study showed significant differences between the pCR and non-pCR groups, indicating their potential as predictive indicators.<br /><br />Further research and validation are needed to confirm these findings and establish PET parameters as reliable prognostic predictors for neoadjuvant IT in early stage lung cancer.
