EP08.02. Prognostic Impacts of Lymphovascular Invasion of Resected Lung Adenocarcinoma after Induction Chemoradiotherapy - PDF(Slides)

This study aimed to investigate the prognostic factors of lung adenocarcinoma after induction chemoradiotherapy from clinicopathological perspectives. The researchers reviewed patients with completely resected lung adenocarcinoma after induction chemoradiotherapy at Kyoto University Hospital from 2007 to 2021. The pathological diagnosis was revised according to the 5th WHO classification of thoracic tumours. The presence of lymphovascular invasion (LVI) and high-grade histological subtypes were analyzed. The effects of these pathological findings on recurrence-free survival (RFS) and overall survival (OS) were evaluated through univariate and multivariate analysis.<br /><br />The results showed that LVI, particularly vascular invasion, was a significant poor prognostic factor for recurrence and survival in lung adenocarcinoma after induction chemoradiotherapy. It was also identified as an important surrogate marker for the effectiveness of multimodal treatment. However, high-grade histological subtypes, such as micropapillary and solid patterns, and the residual rate of primary tumors were not found to be prognostic markers.<br /><br />The study suggested that additional postoperative therapy should be considered based on the presence of LVI, as it indicated a worse prognosis and the possibility of systemic dissemination in locally advanced lung adenocarcinomas. The researchers mentioned that local tri-modality treatment might be inadequate for patients with LVI-positive tumors, and systemic therapy should be added for those at high risk.<br /><br />The study concluded that LVI, particularly vascular invasion, is a significant poor prognostic factor for recurrence and survival in lung adenocarcinoma after induction chemoradiotherapy. The presence of LVI should be considered when determining the appropriate treatment approach for patients. The researchers also mentioned the potential efficacy of adjuvant immuno-checkpoint inhibitors and highlighted the need for prospective trials to investigate their effectiveness in this context.
