EP09.01. Reverse Endobronchial Y-stent Implantation in Selected Cases of the Malignant Central Airway Obtruction - PDF(Abstract)

This presentation discusses the use of reverse endobronchial Y-stent implantation in cases of malignant central airway obstruction in patients with metastatic non-small cell lung cancer (NSCLC). The best method to secure airway patency in these patients is the implantation of a Y-stent. In cases where the right upper lobe bronchi are additionally occluded, it is possible and recommended to perform reverse implantation, with the longer left bronchial arm being inserted into the right main bronchi.<br /><br />The study is a retrospective analysis of the implantation procedures performed at the Department of Thoracic Surgery, Pomeranian Medical University in Szczecin, Poland. The study included patients treated between November 2021 and November 2022 with a tumor in the right lung causing narrowing of the central airways and closure of the right upper lobe bronchi. During the 13-month period, eight successful Y-stent implantations were performed.<br /><br />The study group consisted of five women and three men, with an average age of 66 years. In seven cases, lung cancer was the underlying disease, while esophageal cancer was the reason for one case. The Y-stents used had dimensions of tracheal arm 40-50 mm, right bronchial arm 15-20 mm, and left bronchial arm 30 mm.<br /><br />The reverse implantation of the Y-stent resulted in the restoration of airway patency without any adverse effects. It also better stabilized the main bronchi, preventing the recurrence of obstruction through the progression of hypertrophic changes from the right upper lobe bronchi. Patients experienced rapid clinical improvement, including a reduction in shortness of breath, allowing for further treatment with radiotherapy and chemotherapy.<br /><br />The study concludes that reverse implantation of Y-stents can provide better stabilization of narrowed airways in metastatic NSCLC. This non-standard use of Y-stents demonstrates their adaptability to endobronchial conditions changed by cancer.
