EP09.01. Surgical Treatment in Non-small Cell Lung Cancer with Pulmonary Oligometastasis; 26 Years of Experience in a Single Center - PDF(Slides)

The study conducted by the Bursa Uludag University Faculty of Medicine aimed to analyze the results of surgical treatment and the role of surgery in patients with non-small cell lung cancer (NSCLC) and pulmonary oligometastasis. The records of 1739 patients who underwent anatomical lung resection for NSCLC between 1996 and 2022 were analyzed retrospectively, and 71 patients (4%) were found to have oligometastatic disease.<br /><br />A multidisciplinary approach was applied to all patients, with discussions held in the multidisciplinary oncology council. Pathologic complete response (pCR) was defined as no viable residual tumor in the resected lung and lymph node tissues. The primary tumor stage was determined using the 8th edition of the TNM classification system.<br /><br />Of the 71 patients with oligometastatic NSCLC, 38 had adenocarcinoma, 23 had squamous cell carcinoma, and 10 had other cell subtypes. Brain metastasis was found in 35.2% of patients, followed by adrenal metastasis in 26.8%, lung metastasis in 16.9%, bone metastasis in 11.3%, and metastasis to other organs in 9.8% of patients.<br /><br />The treatment approach varied, with 43.7% of patients receiving chemotherapy and 14.1% receiving chemoradiotherapy for the primary lung tumor. Segmentectomy, lobectomy, and pneumonectomy were performed on 14.1%, 73.2%, and 12.7% of patients respectively. The median hospital stay was 6.9 days.<br /><br />The study reported a 30.9% 5-year survival rate for all patients. Survival rates varied depending on the site of metastasis, with 31.1% for brain metastasis, 34% for adrenal metastasis, and 45.6% for lung metastasis.<br /><br />A case study was also presented, highlighting a 61-year-old female patient with oligometastatic NSCLC. She underwent neoadjuvant chemotherapy followed by VATS right apical segmentectomy and surrenelectomy. The patient was discharged on the 3rd day after surgery and has been on routine follow-up for six years.<br /><br />In conclusion, the study demonstrated the positive contribution of lung resection to the oncologic outcomes in selected patients with oligometastatic NSCLC. The findings emphasize the importance of a multidisciplinary approach in managing these patients and call for larger case studies to guide treatment approaches specific to different organs.
